Commonly Used Forms

Academic Reinstatement Application When suspended from the university, students may apply for Academic Reinstatement after sitting out for one calendar year. The only exception to this is if students are applying for early reinstatement due to extenuating circumstances. Students must complete the form according to whether they are apply for regular reinstatement or early due to extenuating circumstances. They must provide the necessary documentation and complete the application by the deadlines indicated on the bottom of the form.

Course Repeat Students may repeat any course up to two times in an effort to improve a grade. The MOST RECENT grade earned in a repeated course will be the ONLY grade included for that course in GPA calculations. All attempted coursework and all earned grades will appear on the student’s transcript. Credit will be allowed only once for each course taken and repeated. The repeat policy applies for both MSU courses and equivalent transfer courses. Students should file a Repeat Course Form, and submit it to the Registrar’s Office upon completion of the repeated course. PLEASE NOTE: Individual departments and major programs may limit the number of repeats allowed in courses which apply to the major/minor; specific departments and majors may also determine whether all courses and grades will be used in the GPA computation for program admission or for completion of the major.

Credit Overload This Form is used in situations when students need to take over 18 credits. Depending on the number of credits different signatures are required (Advisor, Department Chair, Dean etc.) NOTE: It is important to make sure students understand they will be paying extra for any credits above 18 (as that is outside the banded tuition).

Graduation Application Students should be applying within one year of graduation (or typically 90 credits). They submit this form to the Registrar’s Office and will receive a graduation evaluation in return which will indicate if they are on track for graduation.
